If you are playing online and wish to win prizes, you must fill out the form at Shop.Gooeycube.com/Prizes. This form is solely to help us with shipping out prizes in a timely fashion after the event. The form must be filled out no later than the end of the event, as it helps us immensely with not having to track down contact information and shipping addresses down the road.
If you have previously filled out this form for another purpose or Gooey Cube event, you do not need to fill it out again unless you have since changed shipping addresses. Physical prizes won will be sent if located in the United States. Digital prizes will be sent if located internationally. Full prize disclaimer is located on the form.

*If at the conclusion of a giveaway or contest, your shipping address is not within the United States, a Gooey Cube digital download of equivalent value will be exchanged for a physical prize. In the case where a winner has access to a selection of prizes, the prize must be chosen within seven days of contest closing or a prize will be selected for the winner. If the prize won is a Gooey Cube product still in development, the prize will be sent to the winner upon completion. If the winner prefers a product of equal value currently available, they may request that item at the time of winning. If the participant is specifically entering a competition for contributing to Gooey Cube material (such as naming contests), participants give permission for Gooey Cube to use winning entries in their world-building materials. If at any point you move or change addresses, please re-submit this form with your new address.
